As of the 1st April 2019 this feature has been withdrawn by the Government/NHS England. We hope to have a new feature to replace this but are unable to give a timescale for this: please check back soon. In the meantime please check with reception to see if your address is in our practice area.

Please print copies of the forms that you will need to submit in order to register with us. Please complete as fully as you can, as we cannot process incomplete forms. Please note some of the forms may not be applicable to you.

Alternatively these forms are available from our Reception.

You will need to provide the following forms of identification:

  • For proof of identity: a valid passport or photo-card driving licence.
  • For proof of address: a recent bank statement, utility bill, council tax bill, or a letting agreement/contract.

Please note that photocopies are not accepted.

Children under the age of 16 whose parent/guardian is registered with the practice or registering at the same time will need to provide either:

  • An original birth certificate or a certified copy
  • A valid passport

Please return the completed forms and supporting documents for all family members wishing to register. We must check all forms carefully so please allow for the time this will take when you choose to come and attend.

You will be registering with the practice rather than an individual GP. Nevertheless you have the right to express a preference to see any doctor when you contact the surgery to make an appointment.

New Patients that register with the practice will be required to have a new patient check, this will be an opportunity to discuss any health problems you may have and help us to provide continuity of care.

Your medical records may take some time to be transferred from your old practice.
