Removal of patients from the practice list

Very occasionally we ask the Clinical Commissioning Group to remove a patient from our list when there has been an irretrievable breakdown of the patient/doctor relationship.

This is done very rarely (less than once a year) and is never done on grounds of a patient’s age, medical condition or because of costly treatment. A patient will not be removed solely because they have made a complaint.

When a patient is removed from the list he/she will be informed by letter with the reasons for this course of action.
